5. SITE INSPECTION

5.1 Site Inspection

5.1.1 Site inspections were carried out weekly to monitor the implementation of proper environmental pollution control and mitigation measures for the Project. A summary of the mitigation measures implementation schedule is provided in Appendix F.

5.1.2 In the reporting quarter, 14 site inspections were carried out. 6 of them were the joint inspections with the IEC, ER, the Contractor and the ET.

5.1.3 No outstanding issues were reported during the reporting period.

5.1.4 All the follow-up actions requested by Contractor’s ET and IEC during the site inspections were undertaken as reported by the Contractor and confirmed in the following weekly site inspection conducted during the reporting month.

5.1.5 Details of observations recorded during the site inspections are presented in Table 5.1.

	4 April 2018	Contractor was reminded to provide sufficient watering on stockpile to suppress dust (Radar Tower).		The item was rectified by the Contractor and inspected on 12 April 2018.
	18 April 2018	Contractor was reminded to replace the NRMM label of mobile crane shall be replaced with proper label color (Zone 3).		The item was rectified by the Contractor and inspected on 26 April 2018.
	26 April 2018	Contractor was reminded that NRMM label on mobile crane shall be replaced with proper label color (Zone 2).		The item was rectified by the Contractor and inspected on 3 May 2018.
	3 May 2018	Dark smoke was emitted from machine plant (Zone 2). Contractor should maintain plant/equipment regularly to prevent dark smoke emission.		The item was rectified by the Contractor and inspected on 10 May 2018.
	10 May 2018	Contractor was reminded that stockpile should be covered properly with impermeable sheeting (Zone 1)		The item was rectified by the Contractor and inspected on 16 May 2018.
	31 May 2018	Contractor was reminded to remove the stockpile to avoid dust (Zone 2)		The item was rectified by the Contractor and inspected on 7 June 2018.
Noise	NA
Water Quality	8 March 2018	Contractor was reminded to take precaution measures at any time of year when rainstorm is likely.	The item was rectified by the Contractor and inspected on 15 March 2018.
	21 March 2018	Stagnant water were accumulating in Portion K. Contractor should complete the pumping system ASAP.	The item was rectified by the Contractor and inspected on 26 March 2018.
	26 March 2018	Sediments and stagnant water were found in the u-channel (Zone 2). Sediments and stagnant water shall be removed regularly.	The item was rectified by the Contractor and inspected on 4 April 2018.
	12 April 2018	Slurry was observed accumulating in the U-Channel (Zone 2). Contractor should clean the U-Channel regularly to prevent accumulation of slurry.	The item was rectified by the Contractor and inspected on 18 April 2018.
	18 April 2018	Contractor was reminded to clean the sediments inside the sedimentation tank regularly (Zone 1).	The item was rectified by the Contractor and inspected on 26 April 2018.
	3 May 2018	Contractor was reminded that stagnant water shall be removed (Zone 3).	The item was rectified by the Contractor and inspected on 10 May 2018.
	16 May 2018	Contractor was reminded to provide concrete bunding at site exits to prevent leakage of muddy water to public areas (Zone 3 and 4).	The item was rectified by the Contractor and inspected on 24 May 2018. (Zone 3)
	24 May 2018	Contractor was reminded to provide concrete bunding at site exits to prevent leakage of muddy water to public areas (Zone 4).	The item was rectified by the Contractor and inspected on 31 May 2018.
	31 May 2018	The mud in wheel washing facility should be removed (Portion I).	The item was rectified by the Contractor and inspected on 7 June 2018.
Chemical and Waste Management	1 March 2018	Oil Stain was found on ground at Zone 2. Contractor should clean the oil stain and dispose the waste as chemical waste.		The item was rectified by the Contractor and inspected on 8 March 2018.
	26 March 2018	Chemical container shall be stored and labelled properly (Zone 1). Drip tray and labels shall be provided		The item was rectified by the Contractor and inspected on 4 April 2018.
